    If you're a family child care home provider or applicant, you need to keep OEC up to date about your program. You need to report:

    • What: Any changes that affect the services you provide that differ from what's stated in your license or application
    • When: Within 5 working days of making the changes

    You may need to upload documents when you're filling out this form. If relevant, make sure to have digital copies of these documents available:

    • lnspection reports for any auxiliary heating devices (like a wood stove, pellet stove, or gas insert)
    • Lead testing results for your water supply
    • Well water testing results (if you have a well)

    Important: Please be aware of Regulation Section 19a-87b-5(c)(2): Non-transferability of the License: A family child care home license is only valid for the residence for which it has been issued. If the provider desires to change the residence, the provider shall immediately provide notice to the Office, submit an application, successfully complete an inspection at the new residence and obtain approval by the Office. You may not operate the family child care home at the new address until all requirements have been met and the Office issues approval to operate at the new address.

  • Emergency caregiver

    This back-up caregiver must be a responsible adult who is at least 20 years old and can arrive at the facility within 15 minutes.

  • Potential for lead paint

  • Homes built before 1978 are more likely to have lead paint, which is dangerous. If your home was built before that year and has peeling paint, we may take paint chip samples for lead testing during your inspection.

  • If you don't know when your home was built, check with:

    • Your town assessor’s office website
    • Town building department

  • Water testing for lead

  • You must upload a lead water test from a state-certified laboratory no more than 12 months before today's date. Find state-certified labs for water testing here. 

    When you test the water:

    • Take a sample from a sink used for drinking and preparing drinks and foods
    • Make sure the water has been standing in plumbing pipes at least 6 hours (Section 19a-87b9i)

  • You need to upload a well water test that was conducted:

    • No more than 12 months before today's date
    • By a state certified laboratory

    The results must show that the water supply is drinkable (potable), adequate, and safe. See Section 19a-87b-9(i) for a list of required well water tests.

  • Swimming pool or nearby body of water

  • Renovation, construction or expansion of the facility

    Notify us about changes to the home — like home construction, additions, or renovations. You must wait for approval from OEC before making any changes to your facility.

  • Note: When there is a swimming pool or any other body of water at the facility there shall be a sturdy fence or barrier, four feet high or higher, which totally and effectively bars access to the water by the children. All entries and exits through such fence or barrier shall have self-closing, self-latching devices or locks.

    If someone moves into your home where you provide child care, you need to tell OEC. This could include family (like a grandchild or in-law), friends, or tenants. If you have more than 1 new household member, just add a row and enter the information.
  • Reminder: Everyone age 18 or older who lives in a licensed family child care home needs to get a background check.

  • Employment outside the home

  • Criminal convictions or Department of Children and Families investigations

    This includes you, staff, or household members.
  • Changes in health status that could affect the care you provide

  • This includes you, staff, or household members. Do you, or any person living in the home have any known medical or emotional disorder or take any medications that would:

    • Pose a risk to children in care
    • Interfere with or jeopardize providing them with proper care
